Summary: The first of our must-watch confined space videos tells the story of Cody McNolty who was working as a welder’s assistant in northern British Columbia. On October 4, 2001, he was helping his father and 4 other workers refit a log barge. The video outlines the tragic account of a confined space rescue that went tragically wrong. Cody was unable to rescue his father who died from lack of oxygen. Four other workers fell unconscious due to the lack of oxygen as well.

Why It Makes the List: This video is a great reminder of the importance of making sure that your workers have adequate health and safety training and know safe work procedures for entering confined spaces as well as confined space rescue training.

Confined space videos: Learn the importance of assessing the risks involved in working in confined spaces

Summary: This next of our “must-watch” confined space videos retells the events surrounding the tragic accident that occurred on October 2, 2007 in Georgetown, Colorado. Five contract painters were killed and another three were injured in a tunnel at the Xcel Energy Company‘s hydroelectric power plant that is situated about 45 miles west of Denver. A chemical fire involving combustible coatings and solvents broke out in the tunnel which was located 1,000 feet underground. The fire blocked the only opening to the tunnel.

Why It Makes the List: The video shows how important it is for a company to adequately assess the dangers of working inside a confined space such as the tunnel in this story and to have properly trained emergency responders standing by for a rescue in case an accident such as this happens.

Confined space videos: It is vital to follow confined space entry protocols

Summary: This next video is a case study produced by the Workplace Safety and Health Council. It follows the events that happened when a marine surveying company was hired to inspect the tanks in a barge that was berthed in a shipyard. When the surveyor arrived, he found that the tanks were closed so he instructed the captain to have them opened that day to ventilate them and he would return the next day. However, the tanks were not opened until the next morning.

Why It Makes the List: This video clearly shows how insufficient planning and communication can cause entry procedures not being followed and can result in fatal consequences.

Confined space videos: You need to follow hazardous materials procedures carefully

Summary: A bulk petroleum storage facility employee and a fireman both died in this next of our confined space videos which took place in Phoenix Arizona. The untrained worker died after entering a 10ft by 20ft toluene storage tank which had become contaminated. He had be instructed to drain and clean the tank. The worker entered the tank without a Self-Contained Breathing Apparatus SCBA and passed out. A fireman who attempted to rescue the worker was killed instantly when the tank exploded and several others were injured.

Why It Makes the List: This video shows actual footage of the tragedy that occurred on November 15, 1984. It shows the importance of implementing a hazardous materials procedure when attempting a rescue.

Confined space videos: Panic can cause even more loss of life

Summary: The final entry in our confined space videos list recounts an incident at ADCO’s Shah Field in 2009. The team in this video went to work, without a confinded space permit in a hazardous area. Tragedy strikes when a worker is told to descend into a pit and fix a rope that had slipped off the valve. Not knowing that the pit was filled with H2S, a highly toxic, odourless gas, the worker went in. He was overcome by the gas and collapsed. His fellow workers attempt to rescue him and one by one they are also killed by the gas.

Why It Makes the List: This video is a tragic tale of how panic can overtake you and without proper confined space training and rescue many lives can be lost unnecessarily.
